Overview

PDE5 inhibitor. FDA-approved for erectile dysfunction and pulmonary arterial hypertension. Very widely used in the TRT and AAS community for multiple purposes: (1) Erectile function and sexual performance during or after cycle. (2) Daily low-dose (2.5–5mg) for cardiovascular benefits — reduces blood pressure, improves endothelial function, and may offset AAS-induced hypertension. (3) Some users report improved muscle pumps and vascularity via vasodilation. The longest-acting PDE5 inhibitor (~36-hour effective duration) — most popular in the community for daily use due to once-daily convenience. Harm reduction: Do not combine with nitrates (severe hypotension risk). Monitor blood pressure. At 5mg/day dose, cardiovascular profile is very well tolerated.

Mechanism of Action

Selective phosphodiesterase type 5 (PDE5) inhibitor. PDE5 normally degrades cGMP in vascular smooth muscle — inhibition maintains elevated cGMP, causing vasodilation. Effects systemic (blood pressure reduction) and tissue-specific (penile, pulmonary).
